Applicants for ‘First Time’ and ‘Renewal’ passports are being served on a first come, first served basis at the Local Government Council Office building in Governor’s Harbour, from Wednesday, October 14th until Friday, October 16th , 2015. There was a robust turn out of applicants on the first day, many of whom were not fully aware of the requirements of the process. These are posted below:
PASSPORT APPLICATION REQUIREMENTS are outlined below:
Ensure that ALL DOCUMENTS are copied and originals are presented.
Order of documents:
(FIRST TIME) Applicants:
Under the age of 18
- National Insurance Card
- Birth Certificate / Registered Affidavit
- Immunization Card
- Primary school and high school letter if 12 years and older
- Three Photos (only one countersigned)
- Application form (countersign at Section 10, also Section 8 filled in by parent)
- Mother’s birth certificate/Registered Affidavit (or Father’s if parents are married with marriage certificate)
- Mother’s passport (Valid)
- ONLY IF BORN AFTER 9TH JULY 1973
- Grandmother’s birth certificate/Registered Affidavit (or Grandfather’s if grandparents are married with marriage certificate)
(RENEWAL) Applicants:
- National Insurance Card
- Birth Certificate/Registered Affidavit
- Passport
- Three Photos (only one countersigned – if change of name, lost, stolen or damaged)
- Application form (countersign at Section 10 – if change of name, lost, stolen or damaged). Also Section 8 filled in by parent if applicant is under 18.
- ONLY IF BORN AFTER 9TH JULY 1973
- Mother’s birth certificate/Registered Affidavit (or Father’s if parents are married with marriage certificate)
- ONLY IF (Grandmother) BORN AFTER 9TH JULY 1973
- Grandmother’s birth certificate/Registered Affidavit (or Grandfather’s if grandparents are married with marriage certificate)
